Player Movement Form

This form is for coaches or team managers to complete when a player is moving up or down a division. This form needs to be completed for every move e.g. if a player is moving up to team 3 for one week then back to team 4, then two forms will need to be completed.

Please complete the form the week the movement is happening as that will be when it is actioned.

Please enter the team name as they appear on Basketball Connect, this is not the division the team is playing in.
